Fair Chances Act Model Forms

 

The Office of Tenant and Landlord Affairs (OTLA) has released three model forms to assist housing providers with compliance with the Fair Housing Act (HB937).

 

The forms are available for download on our Office of Tenant and Landlord Affairs website.

Model Forms:

Landlords can use the model forms provided by OTLA or they can create their own forms that are in compliance with the Fair Chances Act.  These forms are provided as a resource to support compliance; however, you are not required to use our forms for communication with tenants.
