Procorem WorkCenter Request Form

​​​​​​​​​​​​​​Use this form to request access for yourself or to add or remove other users.

Before You Start:

  • Have all required information ready.
  • You cannot save the form and return later.
  • Incomplete forms will not be reviewed.
  • Fields marked with an asterisk (*) are required.

Instructions for Completing the Form

Step 1: Complete Requestor Information

Enter your name, company, role, and contact information.

Indicate whether you are submitting the request:

  • For yourself
  • On behalf of someone else

Note: If you are requesting access only for yourself, when you click Next Page, the form will go directly to Project / WorkCenter details.

Step 2: Select the Request Type

Choose whether you are adding or removing users.

If you are requesting access for others, the form will ask if the request is for more than 5 users. If you select yes:

  • Download and complete the Bulk WorkCenter Request spreadsheet​.(Instructions for completing the Bulk WorkCenter Request spreadsheet are provided in step 4).
  • Upload completed spreadsheet to page 1.
  • When you click Next Page, the form will go directly to Project / WorkCenter details.

Step 3: Enter User Information

This step only applies if you are adding or removing 1–5 users. If you are requesting access for yourself, you will skip this page automatically.

For each user, enter their:

  • Name
  • Email address
  • Property name
  • Role

Step 4: Enter Project/WorkCenter Details

List the projects/WorkCenters the users should be added to or removed from. For more than 5 projects, use the Bulk WorkCenter Request spreadsheet.

Include the following information for each project/WorkCenter:

  • Property/WorkCenter name
  • Property/WorkCenter address

WorkCenter Bulk Requests (More Than 5 Users)

Download and complete the Bulk WorkCenter Request spreadsheet​. You will upload it to the form on Page 1.

Use the spreadsheet if:

  • You are adding or removing more than 5 users
  • The request is for more than 5 projects/WorkCenters

How to Complete the Bulk WorkCenter Request S​preadsheet

Spreadsheet – Sheet 1: Add or remove users from 1 project/WorkCenter

  • In cell 3A, enter the Property Name.
  • In cell 3B, enter the Property Address.
  • Enter all corresponding information for cells 4A – 4F.

Spreadsheet – Sheet 2: Add or remove users from multiple projects/WorkCenters

  • Enter all corresponding information for cells 2A – 2H

Step 5: Submit the Form

Review your entries and submit the form.

You will receive a confirmation email once your request is received. Please allow up to 2 business days for your request to be completed.
